My office is located at the intersection of Chatsworth Avenue and Boston Post Road in Larchmont, NY. When using GPS, make sure you are not misdirected to 4 North Chatsworth Avenue. Some GPS mapping tools automatically translate 4 into 4N, which is at the other side of town by Myrtle Boulevard.
Parking is available on the street as well as in the parking lot behind the building, accessible from Addison Street.
Please buzz 202 on the panel outside the building and I will let you in. Should the door be open for any reason buzz anyway to alert me to your arrival. Have a seat in the waiting area and I will come to get you at our scheduled time.

I advise that you not return the forms via email unless you have encryption software because the confidential information contained therein, which is legally privileged or otherwise protected by law, will not be secure.

Payment
You may pay by check or cash at the time of visit. Payment is due at the end of each session.
Statement
You are responsible for submitting to your insurance company for reimbursement. A Statement will be generated at the end of the month for submission to your insurance company. It contains all the requisite information required for reimbursement. Some insurance companies may require the information to be submitted on a HCFA-1500 form to receive reimbursement. I would be happy to provide this if need be.
Insurance Reimbursement
Your insurance carrier will process the claim as it would a medical claim.
If they have any questions about the services you are receiving, you may have them contact me. Because of privacy laws, you must consent to the release of any information in writing.
